Please ensure you print out the Registration/Waiver Form AND the Medical Form.  Medical forms must be signed by your child's doctor after 
Sept 1, 2021, AND it must be returned to DAYO before March 21st.  Your child cannot participate without this form.  The blue school medical forms are also accepted as a substitute for page 2 of the medical form.
